First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. However, a second appointment will be required to provide treatment.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip and a panoramic x-ray (image) if applicable.
  • A list of medications you are currently taking.
  • If you have medical or dental insurance, bring a copy of your insurance card.
  • To expedite your visit, please complete your medical history and demographic forms online prior to your visit. 
  • Please inform the doctor if you have a medical condition that may be of concern prior to surgery (diabetes, high blood pressure, artificial heart valves and joints, …) or if you are currently taking any medication (blood pressure, insulin, aspirin, anticoagulant therapy, …).

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

A preoperative consultation and examination must be performed prior to being scheduled for surgery. Preoperative anesthesia and surgery instructions are always provided during your consultation appointment.  The same instructions can also be accessed under the surgical instructions tab on this website. All patients under the age of 18 being treated under IV Sedation must have a licensed family member or family approved licensed individual to drive them home at the conclusion of their procedure. 

X-Rays

If your referring dentist or physician has taken x-rays, please request that they forward them to our office. You can also bring them with you to your appointment. If additional images are necessary, they will be taken at our facility.
